How to Add Forms to Your Sites
Use Notion forms by copying the link to your form and then embedding it on a notion page, you can place the form where you want on your site and on the page. This also allows you to store the form responses in a different location on your site or privately in your Notion.
1. Set Up the Form in Notion
Begin by creating a dedicated holding page in Notion specifically for your forms - this approach provides maximum flexibility and organizational clarity.
- In Notion, click the '+' button in the left sidebar
- Select 'Empty page' and name it something like "Forms"
Within this holding page, create a new form from scratch, customizing it to match your specific requirements and preferences
- Click '+' on your Forms page
- Type "/form" and select "Create Form"
- Name your form and customize questions using the Form Builder interface

2. Configure Sharing Settings
Ensure that you ensure that you share the form with anyone on the web option within notion so that Super can see the form and added it to your Super site.
- Click the share form button or change the permissions from the notification on the form itself
- From the who can fill out section, select anyone on the web with link (Public)
Locate and copy the unique form link that will be used for embedding purposes
- In the Share menu for the Form, click "Copy link" icon
3. Add to Your Site
Choose the appropriate location in your Notion page hierarchy for your site and embed the form using the copied link
- Navigate to your desired page
- Paste your form link and select “Embed”
Ensure all changes are properly published to make them visible on your live site
- Click "Share" and ensure "Publish to web" is enabled
Sync the page with your Super site and the form should be available instantly
- Log into your Super dashboard
- Navigate to the page with your newly embedded form
- Sync the page with Notion by pressing the Dashboard Refresh button in the top right above the preview pane
4. Managing Form Responses
Consider these two approaches for handling form submissions:
- Public: Maintain transparency by keeping the forms database visible on your site, allowing visitors to view responses and fostering community engagement
- Keep your forms page within your site's Notion hierarchy.
- This allows the responses to be published and visible in Super.
- Private: Ensure privacy by relocating the forms database outside of your Super site's page hierarchy in Notion, maintaining exclusive access to response data
- In Notion, move the forms page to a different area in your Notion - the Form will still need to be shared so Super can see it.
- The form remains embedded on your site while the responses page is private.
Optional: Hide the Forms Page
For a cleaner navigation experience, you can easily conceal the forms page from your site's menu by accessing the design layout section and toggling off the Notion page link visibility
- In Super, go to Design > Layout
- Scroll to the section Notion Page Links
- Click Hide to hide Notion page links
If you have moved your Forms builder and responses page to another area of Notion, then that will not appear on the live site — only the embedded form will appear.
